A $5 coupon for a $4.99 item will result in a $4.99 coupon value.” This means that you won’t receive overage if your coupon exceeds the purchase price, but the coupon should instead just adjust down.
Oral-B digital coupons often exclude the inexpensive Healthy Clean variety, but this particular coupon does not. I scanned the toothbrush with the app, and it did show that the coupon attached. It also lists the coupon on the Oral-B Healthy Clean product page.
