Visit ActivateRewards.com/Wellbeing, enter your information, and submit the image of your full receipt by Mar. 16, 2024.

Receive Reward: Submissions are reviewed within 2 - 5 business days. Once your submission has been validated, you'll receive an email with a link to choose your Reward.

There is a limit of one (1) Sweepstakes entry per person per day during the Sweepstakes period, regardless of entry method, using only one (1) email address. One (1) Sweepstakes entry per receipt submission.
